Consultation will be provided to assist the older clients and family/friends to understand the services that Senior Care Management might provide for the client. As a part of the discussion, the client will be asked to share information about their perception of their own needs and how they think these needs could be met. If the client wants to proceed to use the services of a Care Manager, an assessment of the client’s situation will be completed. (There will be no charge for the first hour of consultation).
